"Heart attack in a bag” is a meal loaded with potentially artery-clogging food. “Heart attack in a bag” is cited in print since at least January 1995 and first described fast food meals (such as McDonald’s), but gradually expanded to other bagged foods (such as donuts, potato chips and popcorn).
Similar expressions include “heart attack on a plate” (since April 1988), “heart attack on a bun” (since February 1992), “heart attack on a stick” (since August 1995), “heart attack in a can” (since February 1998) and “heart attack in a bottle” (since July 1999).

Ah, yes, the McD’s in Louisiana with their Double Les Royals avec fromage (DOUBLE! UGH!) and Double Les Big Macs (with *four* patties) I’m sure contribute much to the excess tonnage we saw while on the road in that state. I’d never seen such things here. Add in the super backhoe-sized scoop of pommes frites and a gallon of sugary coke, and it’s a heart attack in a bag.

i am a big in and out fan(its a shame its only a california thing) also jack in the box is good because i love their curly fries and oreo shakes.also they have a wider selection of food then most fast food places like pitas ,good salads,even terriaki(sp?) bowls and good cheese bacon potato wedges.plus the heart attack in a bag...the triple ultimate cheeseburger!!yum!
