The 2019–20 coronavirus pandemic resulted in some requirements to wear face masks. “Muzzletard” (face mask + retard) represents someone who wears a mask, or is a fanatic that everyone wears a mask. “Muzzletard” could also mean the exact opposite (someone who is a fanatic that everyone does not wear a mask).
